Launched in December 2019, the JOIN Guidelines promote the importance of First Aid in daily situations at home, at work and in daily life.

In the event of an accident or medical emergency, First Aid saves lives. Keeping the patient alive and safe until the arrival of expert professional help is a vital part of the chain of care which leads to recovery. The principle of First Aid must be simple skills, clearly taught and capable of being performed in a stressful situation by those with no medical training. This manual is intended to offer a basic level of First Aid for members of the general public. The contents of this manual are offered as a first edition of European First Aid Guidelines for general use. As such they are compatible with current practise, with other specialist guidelines (for example regarding the management of burns) and with the evidence base where one exists.

These guidelines will be updated on a regular basis by the JOIN Clinical Working Group. Aim: Our objectives were to determine the most effective, safe, and feasible first aid (FA) techniques and procedures, and to formulate valid recommendations for training. We focussed on emergencies involving few casualties, where emergency medical services or healthcare professionals are not immediately present at the scene, but are available within a short space of time. Due to time and resource constraints, we limited ourselves to safety, emergency removal, psychosocial FA, traumatology, and poisoning. Cardiopulmonary resuscitation (CPR) was not included because guidelines are already available from the European Resuscitation Council (ERC). The FA guidelines are intended to provide guidance to authors of FA handbooks and those responsible for FA programmes.

These guidelines, together with the ERC resuscitation guidelines, will be integrated into a European FA Reference Guide and a European FA Manual. Methods: To create these guidelines we used an evidence-based guideline development process, based on the methodology of the Scottish Intercollegiate Guidelines Network (SIGN). Results: The recommendations cover FA for bleeding, wounds, burns, spinal and head trauma, musculoskeletal trauma, and poisoning, as well as safety and psychosocial FA. Conclusions: Where good evidence was available, we were able to turn science into practice. Where evidence was lacking, the recommendations were consensus-based. These guidelines provide systematically developed recommendations and justifications for the procedures and techniques that should be included in FA manuals and training programmes.

Johanniter International (JOIN), the international network of charities affiliated to the Alliance of the Orders of Saint John, has published the very first European First Aid Guidelines. These have been developed by JOIN’s clinical working group with participants from several European JOIN members. The guidelines promote the importance of first aid in daily situations at home, at work, and in exceptional situations. Thus, they are not only intended for health professionals, but also for first aiders and for the general public. St John strongly believes that promoting first aid is in the interest of the public. It saves lives and reduces the burden on the public health system.

St John International is supporting its Associations to expand their first aid training in local communities through a social enterprise model. While the guidelines are no substitute for a professional first aid training manual, they set up a European minimum standard in the field of first aid, designed to apply in every European country. Because they provide a solution to some cross-borders health issues, they foster international cooperation. JOIN guidelines are available in fourteen European languages: Danish, Dutch, English, Estonian, Finnish, French, German, Greek, Hungarian, Italian, Latvian, Polish, Spanish, Swedish, as well as in Arabic. They will be reviewed on an annual basis. Feedback and inputs from any health organisation will be considered with great attention.
